Pre-Admission Clinic 

Please note:  The instructions on this page are for pre-admission to surgical services at Baylor Dallas, not Labor and Delivery.

 

The purpose of the Pre-Admission Clinic is to ensure patients are well prepared for their operation or procedure. This includes collecting medical history and physical information to insure the care you are provided is safe and effective.

 

What You Should Bring With You:

To support and provide care for you we will need certain documents when you arrive to the clinic:

 

  • Drivers License or other legal picture identification (federally issued I.D.)  Current insurance information
  • Document from your pharmacy of your medications, including patches
  • Copies of any laboratory information provided to you
  • Copies of any previous surgical or other medical records you may have. 
Documents to Download and Fill-out:

You may download and print the documents to your right at your convenience in the comfort of your home.

 

Please complete and bring them with you during the clinic visit. This will save you time and help improve your experience during your clinic visit. This is not a requirement as these can be filled out at the clinic. If you choose to fill them out at the clinic, please plan for extra time during your clinic visit.

Pre-admit Appointment:

Please call to schedule an appointment for pre-admitting.  

Please Note:  Beginning February 1, 2010 Baylor University Medical Center is moving to an appointments only system and are asking the surgeon’s offices to schedule the Pre-admit Appointment when they call to schedule the surgery. 

 

Patients need to discuss pre-admit appointments with the office scheduler when discussing surgery dates. 

 

Preferably, appointments need to be made 5-10 days prior to the scheduled surgery date in order to allow the doctor to order further tests or arrange medical clearance as necessary.  We will provide test results to the doctor’s office from any tests performed here.

 

 

Day of Admission:

Check-in and Registration

When you first arrive at the clinic you will be checked in and registered by Access Services. They will collect identification and insurance information.

 

Meet with Admissions Nurse

Next you will meet with a nurse to get vital signs and review any pertinent medical history you have. Orders may be given to have additional tests done which may include some or all of the following depending on your surgical procedure and current health condition.

 

  • Various vital signs taken (blood pressure, heart rate, oxygen saturation
  • Blood drawn
  • EKG/ECG (more details measurements of your heart)
  • X-rays
  • Pregnancy testing (for everyone’s medical safety)
  • Other possible tests as determined by your care providers

 

The average appointment takes 1-11/2 hours. 

 

 

Parking:

One parking voucher will be provided for the pre-admit visit and it is accepted by valet parking. 

 

Valet parking at the Washington Street Medical Plaza entrance is available from 6:30a until 11:00p. 

 

You may also park in lot 8 under the Wadley/Barnett towers. 

 

 

Frequently Asked Questions:

Q.  What do I need to do to prepare for my surgery tomorrow?.

A.  Don’t eat or drink after midnight.  Ask your physician about taking any medication the day of surgery. Remove ALL makeup, jewelry, fingernail polish before arriving. Uncomplicated clothing is also recommended.  Please leave valuables at home; you may need to bring cash or a credit card if you’ve been informed you owe a co-pay.

 

Q.  I need directions and where do I park when I get there?.

A.  We recommend for your convenience you use the valet parking on Washington Street. Directions can be found on the link(s) below. Valet parking is not available at 0500-they don’t open until 0630.

 

Q.  What day and time will my surgery be?

A.  Your Surgeon or their office will provide you the scheduled date and time of your surgery.

Q.  Should I allow and plan for extra time getting to the clinic?

A.  Yes, traffic, weather and construction in Dallas along with other conditions may make you late for your appointment. Also, plan to spend extra time at the clinic because other patients will most likely experience the same conditions which can cause congestion and delays at the clinic.

 

Q. Do I need to fast (nothing to eat or drink) prior to my pre-admission  appointment?

A. No.
